New Company Information

If your company would like to do business with Express HEALTH, please print this form and fax it back to us. We will set up a new account for your company. All you would need to do from here would be to print out the Medical Authorization form below, fill it out and send it with your employee.

Medical Authorization

The medical authorization form ensures us that you have given permission for Express HEALTH to issue medical care to your employee, or authorizes Express HEALTH to perform a pre-employment procedure.
