MEMBERSHIP DISCOUNT PROGRAM

Who can Join?

Anyone! This program is designed for those who are uninsured, in between jobs with no insurance, or underinsured (One may have catastrophic coverage, but needs urgent or family care)

What services are covered?

As it is hard to list everything Express HEALTH can provide, it is easier to say that all members are entitled to all services for which Express HEALTH can perform in side our facility. Ancillary tests like MRI’s, Ultrasounds, CT Scans, and stress testing, just to name a few, are not performed within the Express HEALTH’s facility, and would not be covered under the membership.

When can I join?

You may join at anytime. You can even join the same day that services are rendered.

Do I need to sign a long term contract?

Express HEALTH does not believe in long term contracts. We feel that we provide high quality health care at an affordable rate, and are confident that you will see the value of this program and stay with it for as long as you need our services. However, if you purchase a membership during an Express HEALTH visit or just prior to visiting an Express HEALTH center and give notice of cancellation within the first 45 days, your refund will be given (less a 50% NON refundable enrollment fee).

What does it cost?

We are glad you asked this question! We are excited to say with the Express HEALTH "Membership Discount Program", you will pay only $110 per year and receive benefits for yourself, or $210 per year to receive benefits for all legal dependents living within the same residence.

Do I need a credit card?

NO. Although Express HEALTH would prefer that you use a credit card (Visa/MasterCard), it is not necessary. We also accept cash and debit cards. If you are using cash, Check, or a debit card, Express HEALTH requires that you pay for three months in advance. At the end of three months we will send you a reminder letter to see if you are interested in continuing your membership with us.

Who is covered on my membership plan?

If you purchased a single membership, then only you are covered. If you purchase a family membership then any and all legal/immediate family members are covered. You can not sign up a family member who lives outside your residence.

What is NOT covered with my membership?

Again, this is not insurance and your membership will not cover any tests, services, referrals, or consultations that are performed outside our facility. Members cannot use their membership benefits on work related injuries or any additional promotions Express HEALTH may offer.

What if I want to cancel?

You may cancel at any time. Your cancellation must be received at least 5 business days prior to your next scheduled payment date. You may cancel within the first 30 days for a full refund, less the $10 application fee. However, as stated above, if you purchase your membership during an Express HEALTH visit or just prior to visiting an Express HEALTH center and give notice of cancellation within the first 30 days, your refund will be less the discount given at the time of service. This situation may require you to pay back a portion of the discount you obtained at the time of service.

Are there other Doctors/Offices that are part of this program?

No. Currently Express HEALTH is working with other providers in the McKinney area to offer discounted rates. We will update members as they become available.

Can I add someone later?

Yes. If your situation changes you may add a spouse or child(ren) later.

Can I get my medications/prescriptions here?

No. Express HEALTH does not employ a pharmacist on staff. However, our medical providers will always consider writing for a $4 prescription as the first choice if your medical needs require it.
